New Patient Registration

registrationWhen you register at the practice we will give you a registration form to fill in and we may make you an appointment for a new patient medical. At this appointment we will ask you questions about your medical history and discuss any concerns you may have about your health.

The practice does not discriminate on the grounds of :-

Practice Area

The practice has a specific registration area, a detailed registration area map is displayed in reception and we will register any patient who lives in this area, you may be asked for proof of address. The practice does not discriminate against any race or religion. If you move but are still in our practice area we will continue to be your GP. However, patients who have been removed from the practice list for threatening or abusive behaviour, or for abusing the appointment system, will not be reregistered.
