Care Navigator Team

We have recently re-branded our reception team (previously called Receptionists) as Care Navigators in line with their level of training and expertise.
Care Navigators can have a range of 'day jobs' – in our practice they are fully trained and their role is to actively listen, to signpost people to sources of help, advocacy and support, and to help patients play an active role in managing their own health
They may signpost patients to local services such as the Community Pharmacy scheme (where the patient will have a dedicated appointment with a local pharmacist), to the Social Prescribing Link Workers or to book an appointment with one of our GPs, Nursing team, Health Care Assistants or Practice Pharmacists.
