Practice Nursing Team

We have three very dedicated & experienced Practice Nurses at the surgery who provide the following services:

Long Term Conditions

asthma

Chronic Obstructive Pulmonary Disease (COPD) & Asthma reviews

An annual check up for both of these respiratory conditions is recommended for all patients for monitoring, advice and inhaler use. This is also an opportunity for you to discuss any issues you may be having with an experienced nurse.

You may be asked to complete an online E-consultation form prior to your annual review and these can be accessed via the following links:

Diabetes

Our  nursing team offer an annual Diabetes check up which is a two-step process:

1. An initial "Data gathering appointment" with the Healthcare Assistant which will involve a blood pressure check and your bloods being taken.

2. A 30 minute "Data sharing appointment" during which one of the practice nurses will go through your results, discuss lifestyle measures, your current medications and work with you to ensure your diabetes is as well managed as possible.

For those patient's requiring more specialist diabetes support in the community the "Intermediate Diabetes Specialist Service (IDSS)" team also offer appointments at the practice.

Your  GP or the Practice Nurse can refer you to this if and when needed.

Immunisations

Please contact the surgery to check if your child is up to date with their immunisations.

Many healthcare services have been put on hold during the corona virus pandemic but childhood immunisations have continued because they protect against preventable diseases such as Whooping cough and Measles which are 6 times more infectious than covid-19.

We therefore encourage all parents to ensure that your child(ren) remain up to date with their vaccinations.

Our practice nurses will take all possible precautions to protect you and your child by wearing appropriate PPE and cleaning rooms and equipment regularly.

If you have any question or concerns regarding immunisations, our nursing team are happy to discuss.

Well Women Clinic (WWC)

Please note that our WWC continues to run on a Tuesday afternoon however with a different format to what our patients may be used to.

The clinic is no longer a walk-in service due to the current & ongoing Covid-19 pandemic. Changes have been made to reduce the spread of the virus and to ensure we can observe social distancing in the waiting room with fewer people in the surgery at any one time.

NHS Health Checks & "High Risk" Reviews:

We usually offer 5-yearly NHS Health Checks to all patients aged 40 - 74yrs. Please note that these are NOT currently taking place due to Covid-19 priority work.

These checks can identify patients who might be at higher risk of heart attacks, strokes, diabetes and kidney problems so that we can offer early preventative advice. Please book an appointment with the Health Care Assistant.

We also offer annual High Risk of Diabetes & High Risk of Cardiovascular Disease Reviews also with the Health Care Assistant to any patients identified as potentially benefitting from regular checks.

All patients over the age of 75 years are invited to attend an Over 75 Health Check with the nurse or HCA every 2 years. This gives you an opportunity to consider your general health and well-being.
